FLUORIDATION:
Dental caries is one of the most common diseases worldwide. There have been many treatment modalities that are employed in the treatment of dental decay. However, emphasis must be laid on preventive therapy as it can stop the disease from occurring in the first place. Preventive dentistry concentrates on eliminating the causes of oral disease, thereby preventing the onset of the disease. The importance of fluoride in dentistry has been reiterated time and again. The beneficial effects fluoride has made them an indispensable part of dental practice.
Fluoride and its effect on teeth:
Fluorine is a member of the halogen family of elements. It is the most electronegative and reactive of all the halogens. The world fluorine is derived from the latin term “Fluore” meaning “to flow”. Fluoride and its selective action on hard tissues in the body can be significant in prevention and control of dental caries. Fluoride can protect the teeth in two ways – Systemically and Topically. Systemic fluoride therapy is carried out by fluoridation of drinking water, milk, salt and by other means. Initially it was believed that the action of fluoride was exclusively systemic. The middle of the 20th century saw a lot of evidence of the beneficial effects of topical fluoride exposure.
What are the types of topical fluoride materials available?
There are different forms of fluorides available for dental use such as:
- Fluoride Varnishes
- Fluoride Foams
- Fluoride Rinses
- Fluoride Gels
The fluoride varnishes and gels are more commonly used in comparison to other forms of topical fluoride application.
Fluoride Varnishes:
Fluoride varnishes are protective coatings of fluoride painted on to the child’s teeth to help prevent the onset of dental caries. They can also be painted on to teeth that have already undergone carious degeneration as fluoride varnishes help in slowing down the progression of dental caries. Fluoride varnishes usually contain 5% sodium fluoride.
Indications of fluoride varnishes:

Advantages of fluoride varnishes:

Disadvantages of fluoride varnishes:

Fluoride Gels:
Fluoride gels are viscous fluoride vehicles that are used with foam trays for topical application on the teeth. The different kinds of topical fluorides used are sodium fluoride, stannous fluoride, acidulated phosphate fluoride and amine fluoride. The fluoride gel is loaded onto the foam tray and it is kept in the patients mouth for a period of 2-4 minutes. This is a very effective method of fluoride application as the concentration of fluoride is high. Care should be taken to prevent ingestion of the gel by paediatric patients. 1.23% APF gel is used patients without any tooth coloured restorations and 2.0% neutral sodium fluoride is used in patients with composite restorations or sensitivity.
Advantages of APF Gel:
- Only 2 applications per year
- Deposits fluoride deep into the crystalline structure of the tooth
Disadvantages of APF Gels:
- Acidic, sour and bitter
- Cannot be stored in glass containers
