The OSHA and CDC guidelines recommend scrubbing of dental instruments right after use. This is to ensure a better level of disinfection and reduce the load of microorganisms. This is carried out by the dental assistants. However, if the assistant is not present it is undesirable for the dentist scrub the used instruments, when they could be treating more patients.  The invention of the ultrasonic cleaner machine has been a great advancement in the field of dentistry. It reduces the work of the dental assistant and ensures acceptable levels of post usage disinfection of instruments. Also known as ultrasonic baths, these machines work on the principles of cavitation that is produced by sound waves generated from electrical energy.


How do ultrasonic cleaners work?

Digital ultrasonic cleaners utilize ultrasound waves to clean dental instruments placed in them. Varying levels of ultrasound frequencies are present, but typically it falls between 20 kHz to 400 kHz. As the sound waves produced by the ultrasonic cleaner move through the cleaning solution they create millions of minute bubbles. Alternating low and high pressure waves generated by high frequency sound waves cause the formation of these bubbles. During the low-pressure phase, the bubbles grow from a microscopic size. Subsequently during the high-pressure phase these bubbles are compressed and they implode. Millions of these microbubbles implode simultaneously on the dental instrument placed in the ultrasonic bath. The implosion of these microbubbles, causes disruption of the chemical bonds which hold the debris on the surface of the dental instruments. As a result, the debris are removed from the instruments. This process is termed as Cavitation. The ultrasonic cleaner aids in cleaning dental instruments by using a high and low frequency waves to agitate the liquid creating rapid formation and collapse of bubbles. The results from using ultrasonic cleaners are more effective than conventional hand scrubbing with a scrub brush.


Testing your ultrasonic cleaner:

Ultrasonic cleaning devices must be tested on a monthly basis, to ensure proper functioning. There are certain tests that can be carried out to check the effectiveness of the device in cleaning.



Degassing is removal of small suspended gas bubbles and dissolved gas which form from the liquid as a result of consistent usage of the device. The dissolved gas should be removed from the unit as it reduces the efficiency of cleaning. The gas migrates into the cavitation bubbles when they are formed and prevent them from imploding violently. The gas bubbles in turn, provide a cushioning effect to the imploding bubbles which also reduces its efficiency in cleaning.

Degassing can be carried out by raising the temperature, adding cleaning agents and opertating the unit for 10-30 minutes. However, this depends on the size of the tank and the nature and concentration of the liquids. In a completely degassed liquid, small bubbles will cease to rise to the surface of the liquid.


Aluminium foil test:

Testing the ultrasonic scaler with aluminium foil is one of the most reliable methods. This is a simple test to test cavitation of the device.

  • Place a piece of aluminium foil vertically in the cleaning bath down to the bottom.
  • The foil should span the opposite corners.
  • The usual immersion time for aluminium foil is 1 minute
  • The foil should contain patterns of indentations or holes at the end of the process
  • This shows that even cavitation occurs in the device.
  • Inconsistent patterns and dead zones indicated inconsistent cleaning


Glass Slide Test:

The glass slide test is another simple method of testing the efficiency of the ultrasonic scaler.

  • The frosted portion of the glass slide is wetted with tap water and an “X” is drawn using a No. 2 pencil from corner to corner
  • The tank is filled till the fill line and the frosted end of the slide is immersed into the solution
  • On turning the unit on, the lead “X” should disappear almost instantly.
  • All other lead should be removed within ten seconds.


Tips while using ultrasonic baths:

  • Keep the lid on the unit the entire time when it is functioning, to limit aerosols and reduce the noise level.
  • Do not overfill the ultrasonic bath as it would make it less effective in cleaning
  • When the fluid becomes visibly cloudy, dirty or after subjecting it to multiple uses, a fresh solution should be used before the next use. Busy practices might require multiple changes of solutions.
  • Do not use chlorides or corrosive fluids. Use no-foam chemicals. Do not use fluids that are not recommended by the manufacturer. The type of the fluid is as important as the amount.
  • After cleaning the instruments using the ultrasonic bath, rinse them to remove any chemicals or residual debris
  • At the end of each day, the unit should be devoid of fluid and flushed with clear water. It should be dried, keeping the lid off, for the entire night.
  • Do not keep the unit near the sink to fill water. The electrical items if wet, might cause malfunction of the equipment.
  • Placing items directly at the bottom of the unit without using a basket is not recommended. It can impact the transducers present, which limit cavitation.
