ROOT CANAL SHAPING IN ENDODONTICS:

Root canal therapy is one of the widely performed dental procedures. BIOLOGICAL OBJECTIVES:

  • Confine the use of instruments within the root canal space to maintain its spatial integrity
  • Avoid pushing debris past the apical constriction
  • Remove all potential irritants from the canal system
  • Determine the exact working length and clean and shape the canals during the first visit
  • Create sufficient space in the coronal half of the canal to facilitate irrigation and debridement

 

MECHANINCAL OBJECTIVES:

  • Create a continuously tapering conical form to the canal preparation
  • Prepare a sound apical dentin matric at the cementodentinal junction
  • Prepare the canal to taper apically, with the narrowest portion being apical and a wide coronal portion
  • Restrict preparation till the apical constriction and not beyond it
  • Confine cleaning and shaping to the root canal system and maintain the spatial integrity of the canal.
  • Remove all residue of cleaning and shaping the canal to maintain the patency of the apical foramen

 

What are the instruments used in biomechanical preparation?

Over the years, intensive research and development have resulted in the invention and production of various instruments to effectively shape the root canal. Materials such as nickel titanium, due to their elastic property have been extensively used to make instruments for shaping. The following are the instruments used in root canal shaping:

 

Endodontic Files:

The most commonly used instruments to shape the root canal are endodontic files. In the past, files were manufactured at the whim of the manufacturers without any specifications. Ingle and Levine were the first to suggest an increase in diameter as the size progressed, but maintaining a constant taper of all blades regardless of file size.

Endodontic files are available as K files, H files, S files, NiTi Hand Files and NiTi Rotary Files. NiTi Files showing excellent flexibility are highly useful in shaping curved canals. Files are used mostly in a filing motion. Other instrumentation motions include reaming, watch-winding and the balanced force technique.

 

Dental Reamers:

Reamers are used for the same purposes as files, however they differ in certain properties. The name ‘reamer’ comes from the reaming motion, with which it is intended to be used. The differences between files and reamers are tabulated below.

Gates Glidden Drills:

Gates Glidden drills are rotary cutting instruments intended to be used at 750-1500rpm, to shape the root canal. They can be used in both the step back technique and the crown down technique of biomechanical preparation. These drills are particularly used in enlarging the coronal third of the root canal and removal of the lingual shoulder which may hinder gaining a straight line access. They are side cutting instruments with safety tips to reduce the risk of accidental perforation of the floor of the pulp chamber or the furcation.
