ANCHORAGE:

In orthodontic therapy, the movement of teeth is brought about by the forces generated by the active components of the removable of fixed appliance. These forces are derived from certain anatomical areas which act as anchorage. One of the greatest orthodontists of the 20th century, Dr T.M. Graber defines anchorage in orthodontics as “the nature and degree of resistance to displacement offered by an anatomical unit when used for the purpose of effecting tooth movement.” In simpler words, it can be described as the resistance to unwanted tooth movement.       

 

What are the various types of anchorage in orthodontics?

Anchorage used in orthodontics can be classified under various heads. This classification is based on the manner of force application, the jaws involved, the site of anchorage and the number of anchorage units.

 

Based on the manner of force application anchorage can be classified as-

  • Simple Anchorage
  • Stationary Anchorage and,
  • Reciprocal Anchorage 

 

According to the jaws involved in anchorage –

  • Intermaxillary anchorage and,
  • Intramaxillary anchorage.

 

Based on the site of anchorage –

  • Intraoral
  • Extraoral and,
  • Muscular

Extraoral anchorage can be further divided into Cranial, Occipital, Cranial and Facial anchorage.

 

Based on the number of anchorage units –

  • Primary Anchorage
  • Compound Anchorage and,
  • Reinforced Anchorage

 

Molar Bands and Attachments:

Molar bands are metal rings that are attached to specific teeth to act as anchors and to fix various attachments used in orthodontic therapy. The factors to be considered when choosing a tooth to serve as an anchor are –

  • Root Form
  • Root Size
  • Number of Roots
  • Length of roots and,
  • Inclination of roots

 

Teeth with more number of roots are usually employed for anchorage. The first maxillary molars (16 & 26) are hence the most commonly used anchor teeth. Attachments such as buccal tubes and hooks are bonded, welded or soldered onto the molar band to engage the archwire and elastics used.

 

What are TADs?

Temporary Anchorage Devices are sometimes used when anchorage from the teeth is not sufficient and to prevent anchorage loss. They are devices that are temporarily fixed to the bone to enhance anchorage directly or indirectly. Mini implants are employed in orthodontics as TADs. The mini implants have three parts namely the head, neck and screw. They are made from three classes of materials grouped as biotolerant (stainless steel, chrome cobalt alloy), bioinert (titanium, carbon) and bioactive (vetroceramic apatite hydroxide, ceramic oxidized aluminium). However, grade V titanium, an alloy of titanium, aluminium and vanadium (Ti6Al4V) is the material of choice. The mini implants can be subperiosteal, transosseous or endosseous based on their location. The most commonly employed ones are the endosseous implants. Brands such as D-Tech and Moriz manufacture quality micro screws to be used as temporary anchorage devices. 

 

What are the Orthopedic appliances used in anchorage?

Extra oral sites can be used to increase the strength of anchorage when the resistance offered by intraoral sites are not sufficient. The cranium, cervical region and facial bones can be used as extraoral sites of anchorage. There are a number of devices that can be used on conjunction with removable or fixed orthodontic devices to enhance anchorage such as-

  • Head gears
  • Chin cups
  • Protraction face masks

These appliances can produce skeletal changes through the application of orthopaedic forces. Intermittent forces of high magnitude are delivered by the orthopaedic appliances which are directed the basal bone via the teeth. This results in modification of the bone apposition pattern.
